On-site fitness opportunities Employee meal program Education assistance Responsibilities: Build long-term relationships with residents and their families with a person-centered focus. Create and maintain atmosphere of warmth, personal interest and positive emphasis Act as a liaison between hospitals rehabilitation centers and RiverWoods Healthcare to coordinate smooth admissions of RiverWoods residents into healthcare, including room management. Provide residents with information concerning resident rights, living wills, power of attorney documentation, advanced directives, RiverWoods Lodge Handbook, Long-term care Ombudsman information and other pertinent information upon a resident’s admission to healthcare. Complete all admission paperwork and consents in accordance with federal and state requirements as well as RiverWoods established policies and procedures. Participate in the development of a written, interdisciplinary plan of care for each resident that identifies social, emotional, psychosocial and spiritual needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social worker intervention. Ensure or provide therapeutic interventions to assist residents, and their families in coping with their transition and adjustment to the healthcare facility, including their social, emotional, psychological and spiritual needs. Qualifications: Minimum 4-year college degree and BSW required, with a preference for MSW degree Minimum of 5 years of experience in a long-term care setting, healthcare or related field For assistance with your application call 603.658.1541 or email bbaadetrwg.org directly.
