Job Description Job Description Hybrid Health Worker II – Chronic Care Management and Remote Patient Monitoring Location: Hybrid (Onsite and Remote) Position Summary: The Hybrid Health Worker is responsible for providing comprehensive care coordination for chronically ill patients through both onsite visits and remote patient monitoring (RPM). This role ensures continuity of care by managing patient communication, monitoring health data, and collaborating with interdisciplinary healthcare teams. The position requires excellent communication skills, clinical knowledge, and proficiency with digital health technologies. Join our growing team of Health Workers and create the life you want while doing meaningful work caring for our aging population · Be your own boss · Set your own schedule · Work when and where you want · Improved work life balance · Earn extra income Responsibilities: · Transport, assist and care for clients going to medical appointments, errands, and recreational activities. · Provide emotional support and patient advocacy. · Communicate ongoing care results and updates to relevant parties as appropriate. · Collaborate with clients and families for the best care opportunities as needed. · Assist clients in maintaining a healthy and safe living environment. · Monitor patient health data through remote patient monitoring platforms and escalate concerns as needed. · Conduct routine patient assessments through telehealth and in-person visits. · Develop, implement, and update personalized care plans in collaboration with primary care providers and specialists. · Educate patients and caregivers on condition management, medication adherence, and self-care practices. · Track patient progress, document interactions, and maintain accurate records in the electronic health record (EHR) system. · Communicate with healthcare providers to coordinate interventions and ensure timely care delivery. · Assist with scheduling follow-ups and referrals as necessary. Required Qualifications: · Well maintained and reliable personal 4 door vehicle/clean driving record · Previous experience in senior care or other related fields · Healthcare certification (ex, CAN, RN, LPN etc.) · Compassionate and caring demeanor, ability to build rapport with clients · Coachable, go-getter, entrepreneurial minded, self-starter and team player. · Must be experienced in updated technology for patient monitoring systems. · Willing to travel to all major metro hubs in Virginia (Charlottesville, Richmond, Virginia Beach, Norfolk etc.) · Familiarity with remote patient monitoring technology and telehealth platforms. · Excellent organizational and time-management skills. · Strong communication and interpersonal abilities. · Proficient in using EHR systems and other healthcare management software. · Ability to work independently and collaboratively in a hybrid environment.
