RN Care Manager

Additionally, you will review patient records and collaborate with the interdisciplinary team to gather comprehensive information for effective communication with payors, aiming to prevent denials. The RN Care Manager role will also involve participating in interdisciplinary rounding to identify areas for improvement and facilitating communication between patients, families, and social work to ensure safe and appropriate discharge plans. The ideal candidate will bring over three years of diverse RN experience, with a background in critical care, home care, or case management preferred.
