Job Title: Social Worker 2 Job Specialty: Health Care Job Duration: 52 weeks Shift: 8:00 AM - 5:00 PM Regular Shift Guaranteed Hours: [Not specified] Experience: [Not specified] License: [Relevant license info here, if applicable] Certifications: [Relevant certification info here, if applicable] Must-Have: [Any essential skills or requirements here, if applicable] Job Description: • Take referrals from primary care providers and work with the care team to provide social support navigation for social determinants of health, including food insecurity and housing insecurity. • Compile and maintain a resource list for resources related to social determinants of health, including eligibility criteria, referral process, and contact information. • Collaborate with primary care nurses and providers. • Provide in-person or remote social needs screening/assessment for patients referred by nurses or providers. • Coordinate or inform about available social services resources, such as housing, clothing, food, and mental health services. • Work alongside other social workers to identify both patient and community resources. • Conduct case management activities, including working with hospitals for discharge planning, follow-up, and education. • Assist in obtaining patient records and securing needed medical equipment through community partners. • Conduct follow-up on care plans and assist patients who are lost to follow-up or overdue for care in returning to care. • Support specialty referral navigation by scheduling, coordinating, and tracking non-specialist and imaging referrals. • Assist with obtaining patient records from specialists and imaging centers. • Compile and maintain a resource list for specialty referrals, detailing eligibility criteria, referral process, cost, and contact information. • Help patients locate and access low-cost prescription options, including patient assistance programs and discount retailers. • Assist with patient assistance program applications and serve as a liaison between patients and drug companies. • Help patients with applications for relevant programs. • Participate in other regional initiatives with a social work component as needed. • Document in the patient's record, update consults, and notify providers and/or clinical staff as necessary. • Provide patient education or find suitable educational resources. Expectations may include: • Completing onboarding and orientation. • Participating in regional office and primary care clinical meetings as requested. • Attending provider meetings as requested. • Participating in health councils and other community meetings to build relationships with social service agencies and promote health department services. • Identifying barriers to care experienced by patients and seeking ways to address them.
