CNA PER DIEM- ASSISTED CARE Position SummaryFor over 35 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65 by offering a host of hospitality, wellness and health care services. The Care Partner will provide care for Residents and administer necessary unit tasks and functions in compliance with North Hill's policies, procedures, and standards. The individual will partner with Residents to provide Resident centered care, including ADL's and the preparation and serving of meals.Job Specific DutiesReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Partners with Residents to provide Resident centered care, including bathing, dressing, grooming, nail care, shaving, and toileting• Assists in transporting residents to and from social programs• Responds to Resident requests for assistance and care• Leads and participates in small group programming activities• Answers personal emergency response system (PERS) calls promptly• Provides medication reminders per the assignment and documents accordingly• Reports all changes in Resident condition to the nurse on duty • Understands Resident personal preference and provides support and care in a manner that respects Resident rights, choices, confidentiality, and privacy• Appropriately communicates to the nurse on duty any Resident complaints, concerns or changes in condition• Prepares, cooks, and serves Resident meals. Cleans-up kitchen following mealso Pays attention to and follows the appropriate plan for Resident preferences and food allergies• Maintains the kitchen in a safe and sanitary manner, following all Serve Safe guidelines• Provides light housekeeping in Resident apartments daily as assigned• Provides laundry services as assigned and needed• Completes assignment of Resident Care Needs and documents when completedFollows safety requirements to Include Emergency Management Plan and established Infection Control and Universal Precautions policies and procedures• Follows all Emergency Dining Plans and Life Safety policies and procedures and actively participates in fire and evacuation drills• Completes food and appliance logs per Serve Safe• Reports hazardous conditions, equipment and accidents, and incidents to the supervisor on the shift they occur• Recognizes and reports all allegations of Resident abuse, neglect, exploitation, social media exposure and/or misappropriation of Resident property to the Director of Vista Terrace, the Wellness Nurse Manager or the Nursing Supervisor o Supports the investigation as needed• Provides medications to Residents as required by their service plan • Provides report to oncoming staff• Meets department uniform, appearance, and grooming requirements• Complies with all OSHA safety regulations, applicable local laws regarding health, safety, or environmentSupervisory ResponsibilitiesNoneRequired Skills and Abilities• Ability to maintain confidentiality and strong customer support skills• Ability to work independently with limited supervision• Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public• Work professionally with people from a wide variety of cultural and socio-economic backgrounds• Complies with protection of health information (HIPAA) safeguards• When working in Resident areas, must be able to communicate effectively in English• Attends and participates in in-service educational classes, memory care training, Healthcare Academy and staff meetings as directed Education and Experience• High School Diploma or equivalent• Certified Nursing Assistant in the State of Massachusetts or Home Health Aide Certificate• Maintains CPR certifications• Prior experience as a Certified Nursing Assistant or Home Health Aide is preferred • Proficient with computer software programs and EMRPhysical RequirementsThe physical demands described here are representative of those that must be met by a Team Member to successfully perform the essential functions of this job. • While performing the duties of this job, the team member is regularly required to talk and hear.
