Physical Therapist - Per Diem-Hourly For over 35 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65 by offering a host of hospitality, wellness and health care services. The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation. This is an hourly role not per visit.Knowledge of Medicare Conditions of Participation as well as State and Federal RegulationsProvides patient care and documents treatments in accordance with physicians' orders, agency policy, and patient needsPerforms patient evaluations, selecting appropriate diagnostic material to assess all aspects of care informing the physician of recommended interventionsDevelops a care plan in conjunction with patient, identifying patient centered goals establishing priorities of carePromotes professional growth by attending, presenting formal and informal educational experiences, in-services, conferences, and seminars to teamThis position also assists in the supervision of assistants, Certified Home Health Aide (CHHA), students and interns as assigned according to Medicare Condition of ParticipationOther duties include scheduling patients and staff, participation in discipline specific projects i.e. chart audits, QI data collection, peer review program, student program and Certified Home Health Aide training.
