Job Description
TITLE: Elderly Outreach Coordinator- full time DEPARTMENT: Elderly Services GENERAL DESCRIPTION : This position is responsible for creating new programs and communicating existing programs for the senior community. Linking the senior population both on site and in the community to resources at the city, state, and federal organizations is critical to their wellbeing. Additionally, the position will create and promote new recreational events and linked to physical and mental wellbeing and carry them out whether in house or externally. This position will partner with other City agencies to carry out the role. SUPERVISION RECEIVED: Director of Elderly Services or his/her designee SUPERVISION EXERCISED : None ESSENTIAL DUTIES : Effectively communicates between state and federal agencies and the senior population. Assists in the coordination with the senior community to talk about their needs and concerns. Assists in the directing of seniors to the organizations with plans on improving the individual's quality of life and well-being. Schedules and conducts outreach meetings. Assists in representing the Department of Elderly Services to increase public awareness. Conducts assessments with individuals in the determining of relevant resources and guidance. Follows up with seniors to ensure that the individual's needs are met through their support plan. Maintains accurate record-keeping of confidential information to comply with policies, regulations, and procedures. Provides appropriate customer service in person, on the phone, and by email with proper knowledge and information. Assists in the development of promotional materials and announcements for programs and activities and the dissemination of such materials. KNOWLEDGE, SKILLS, AND ABILITIES : Must possess good communication skills. Must possess strong organizational skills. Must utilize skills in engaging and assisting with others from diverse ethnic and cultural backgrounds, and all social and economic levels. Must demonstrate ability to manage multiple projects. Must display genuine concern for the senior population and tact when dealing with others. QUALIFICATIONS : Minimum bachelor's degree from a recognized college or university in Business Administration or a field related to Human Services coupled with a minimum of 5 years of experience working with vulnerable populations and delivering services to meet their needs. SPECIAL REQUIREMENTS: Valid Connecticut Driver's License. Must have or be able to obtain a certification in CPR, First Aid, and AED Administration. TOOLS AND EQUIPMENT: Telephone, copier, fax and scanning capabilities, senior management software, computer. WORK ENVIRONMENT: Reasonable accommodation may be made to enable individuals with disabilities to perform essential job functions. Minimal physical effort is generally required in performing duties under typical office conditions. Must be able to concentrate on fine detail with some interruption as the noise level in the work environment is usually quiet to moderate. Must be able to remember task/assignment given to self and others over long periods of time. Must be able to perform highly complex tasks requiring independent knowledge, its application to a variety of situations, as well as exercise independent judgment.
