About theservice

We are currently seeking a compassionate and dedicated Support Practitioner to work in an engaging and dynamic work environment in Glasgow, working between our Supported Living Service in Craigton and our Outreach service in the Westend of Glasgow.

The role will provide support for one individual with complex needs, who resides in their own home in the Southside/Craigton area, and one individual with complex needs, living in the family home in the West end. The role will involve enabling both Individuals to access community activities, promote independence and well-being, in addition to attending to their daily personal needs and daily routine.

The young lady we support in the West End has a varied weekly activity programme which includes swimming, hitting the gym, dining out, dance class and is working on developing her independent living skills.

The young lady we support in our 24 hour Supported Living service in Craigton loves going out and about in the local community, dining out, going to parks, shopping and has a passion for Lego. She also loves going on holiday and has her own caravan.

At the core of our approach is effective communication rooted in values, tailoring our support to meet the unique needs of the people we support.

In this role your primary focus will be on establishing trustful relationships and providing meaningful outcomes for the individuals supported., serving as a solid foundation for helping with practical tasks. This involves:

  • Support individuals in their own homes with personal care, housework, administering medication, meal preparation, supporting with finances companionship etc
  • Liaise with families and other professionals in order to provide the best quality care
  • Promote independence and individuality according to each clients Support Plan
  • Record daily tasks and maintaining accurate up to date records

Work as part of a team, effectively communicating with staff team, families and people supported

You are required to work across seven days, which will include shift and weekend work. A flexible and committed approach to ensure consistency and stability for the people we support is essential for both services.
