Job Description
Key Responsibilities:

Patient Assessments: Conduct thorough assessments of individuals' nutritional needs, taking into account their medical history, lifestyle, and personal preferences. Identify any nutritional risks or concerns and develop personalized care plans to address them.

Dietary Advice and Education: Provide one-on-one consultations with patients, offering advice on healthy eating, meal planning, and how to manage specific conditions (e.g., diabetes, hypertension, or obesity) through diet. Educate patients on the role of nutrition in preventing and managing illnesses.
*
Nutritional Interventions: Develop, implement, and review individual nutritional care plans based on patients' medical conditions, needs, and goals. Provide tailored dietary recommendations to support weight management, gastrointestinal health, cardiovascular health, and other chronic conditions.
*
Multidisciplinary Collaboration: Work closely with GPs, nurses, and other healthcare professionals to deliver comprehensive care. Attend multidisciplinary meetings and contribute to care planning discussions to ensure holistic support for patients.
*
Community Outreach and Education: Engage with local communities to raise awareness of healthy eating habits and nutrition-related health issues. Provide group-based education sessions on topics such as nutrition for specific conditions, cooking skills, or weight management.

Monitoring and Review: Regularly monitor and review patients' progress, adjusting care plans as needed. Follow up with patients to ensure that dietary advice has been implemented effectively and make further recommendations if necessary.

Record Keeping: Maintain accurate, up-to-date patient records, ensuring that all assessments, care plans, and follow-ups are documented in line with organizational policies and procedures. Ensure compliance with all legal and regulatory standards related to record-keeping
