Job Description

Job Title: Support Worker (Urdu/Punjabi Speaking)

Introduction: We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will be fluent in Urdu and Punjabi, enabling effective communication with the individuals we support. This role is crucial in providing personalised care and ensuring that our clients lead fulfilling lives in accordance with their Person-Centred Plans.

Objectives: To deliver high-quality care and support tailored to the unique needs of each individual, fostering autonomy and independence while ensuring their safety and wellbeing. You will play a key role in empowering individuals to make choices about their lives and engage in meaningful activities.

Responsibilities:

* Encourage and facilitate personal choice, autonomy, and independence in all aspects of the lives of those we support.

* Assist in planning and delivering recreational, social, and educational activities, providing support in the community as required.

* Maintain accurate and timely records on the Nourish care platform and ensure all documentation is up to date.

* Safely administer medication in accordance with care plans and report any issues to the Registered Manager.

* Support individuals in decision-making, adhering to best practices from the Mental Capacity Act.

* Uphold the rights, dignity, privacy, and respect of the individuals we support at all times.

* Act as a key worker, building and maintaining effective relationships with clients and their support networks.

* Report any concerns regarding health, behaviour, or incidents promptly to management.

* Participate actively in the Person-Centred Planning process and contribute to team meetings and training sessions.

* Ensure adherence to health and safety standards and company policies.

Requirements:

* Fluency in Urdu/Punjabi is essential.

* Previous experience in health and social care is preferred.

* Strong communication skills and the ability to build rapport with clients and their families.

* A commitment to person-centred care and the ability to maintain confidentiality.

* Understanding of the Mental Capacity Act and relevant legislation.

* Ability to work collaboratively as part of a team.

* Willingness to engage in ongoing learning and development.
