Job description: *Physical Therapist - Home Visits:*

Supportive leadership
*Role and Responsibilities:*
* A qualified professional, who directs, supervises,evaluatesandprovidesphysical therapy services to adult patients in the home,following physicians orders.
*Essential Functions:*
* Evaluate patient using diagnostic muscle, nerve, joint,and functional abilitytestsand measures.
* Prepare an assessment of the patients condition at the time of theinitialevaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
* Recommend modification of plan of careto physician,as needed.
* Document clinical notes on the date of the visit into medical record and keep all records and summariesup-to-date.
* Treat patients to address impairments and functional limitations, with a goal ofreachingmaximum functional independence.
* Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic,and durable medical equipment (canes, walkers, wheelchairs, etc.).
* Include and educate thepatientandcaregiver(s)regardingtreatmentplanand goals.
* Attend physical therapy staff meetings and in-service presentations.
* Participate in case conferences.
* Participate in the development and periodic revision of the physicians Plan of Treatment.
* Assureappropriate safetypractices and maintenance of equipment.
* Other duties as assigned.
*Education Requirements:*
* Graduateof an accreditedPhysicalTherapy program.
*Qualifications and Requirements:*
* Current, valid Physical Therapy licenseissuedbyNYSDepartment ofEducation.
* One year of practical experience as a licensed physical therapist.
*Work Environment:*
* The Physical Therapist is primarily ina non-officesetting and may be exposed to outdoor conditions.
*The working conditions are classified as medium work:*
* Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of forcefrequentlyor constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
