Job Description

We are currently recruiting for the position of Team Manager in the Moving On Team within Adult Social Care. The Moving on Team supports Hackney residents aged 18 + who have been in hospital and, due to increased social care needs and/ or concerns around the suitability of their accommodation, are then discharged into interim accommodation, which includes B&B’s, supported living, housing with care or residential and nursing placements.

As the Team Manager you will lead and manage a team of practitioners from varied professional backgrounds and will offer clinical or professional supervision. You will be responsible for ensuring person centered and strengths-based services are provided to all service users and their carers /families. You will manage the performance of the team in relation to key performance indicators and ensure the team's compliance with the Care Act, Mental Capacity Act, Human Rights Act and Safeguarding. You will contribute to FOI requests, complaints and provide information, reports and feedback as required. You will represent the Council in legal proceedings.

As the Team Manager you will have oversight of the interim housing with care properties, ensuring the properties are suitably maintained and lead in partnership working with Provided services and other key internal and external stakeholders to ensure positive outcomes are achieved and the well-being of our residents is promoted.

You will ensure that work is allocated according to all relevant legislative thresholds including Care Act compliance for all service user groups and to monitor caseloads, complexity, and risk. You will ensure effective communication with the team and hold regular meetings with staff.

You will be able to monitor performance and workloads, quality of assessment, support plans and reviews. You will have experience of managing a budget and be able to operate within budgetary constraints and ensure Value for Money in the commissioning of support plans, placements, and services. You will be able to undertake quality assurance audits and ensure team compliance, fulfilling the goals and targets set out in the strategic plan for adults.

As the Team Manager you will attend and where appropriate chair, meetings relevant to the role including team meetings, safeguarding meetings, and wider meetings with the service.
