Job Description

Learning Support Transition Coordinator (Term-time only +4)

This role is to be the key point of contact and build on the strong links with schools, local authorities, and external agencies to support learners with inclusion needs (SEND) and with an Education, Health and Care plan (EHCP) and transition into college, throughout and progression.

To take the lead on individual transition experience visits to ensure a person-centred approach to identifying support requirements, planning resources, including attendance to School and College open day and evening events.

To provide advice and guidance to schools and their pupils about courses and progression pathways offered by the college, ensuring effective links with curriculum teams for planning of required support and adaptations.

Key Responsibilities

  • Work effectively and collaboratively with curriculum teams to ensure sharing of information to best meet learner needs, both pre-entry for transition purposes and progression on programme.
  • Work with external agencies to gather required supporting evidence and information relating to inclusion needs through the EHCP consultation and transition process, and to support High Needs Funding claims. Attend College Open Days, Transition events and School events as required.
  • Working with learners and families, and curriculum teams, coordinate the Education, Health and Care Plan processes both pre-entry to college (EHCP consults) and on programme (EHC reviews) to best meet learner needs and statutory duties.
  • Coordinating caseloads of support, be able to accurately interpret Education, Health and Care Plans, EHCP consultations, and assessment reports.
  • Monitor attendance, retention, and achievement of learners throughout the year to ensure they are on target to meet the outcomes stated in an Education Health and Care Plan or support plan by close liaison with curriculum teams, plan effective interventions for any concerns.
  • To line manage and deploy support staff, oversee performance management to support team members to provide high quality support for learners and the service provided, contributing to the planning and delivery of CPD and training for staff.
  • Produce reports to detail the support arrangements and interventions that have been put in place across the curriculum area/s and the impact on retention, achievement and success, based on the curriculum area records.
  • Continue to build on specialist knowledge for inclusion needs (SEND), EHCP, preparing for adulthood, life and work skills. Ensure continuous development and improvement of professional knowledge
  • Carry out any other duties as may be determined from time to time within the general scope of the post.

Qualifications

Essential Criteria:

  • Level 2 (GCSE A* - C) in English and Maths
  • Degree qualification

Experience

Essential Criteria:

  • Experience of working with 16-25-year olds in an education setting
  • Experience of working with young people and individuals with SEND/Learning Difficulties and/or Disabilities.
  • Experience of working with internal and external stakeholders including schools, parents / carers and support agencies
  • Experience of EHCP processes e.g. Review Meetings and Consultations
  • Experience of providing support to staff and teams
  • Experience of providing support to learners with SEND/Learning Difficulties and/or Disabilities, with an EHCP
  • Experience of development of resources related to support needs/requirements of learners
  • Experience of developing and delivering training and CPD to staff
  • Successfully supporting a culture of team work within a multi-disciplinary environment

Desirable Criteria:

  • Coordinating and chairing of SEND/EHCP related meetings and person-centred reviews
  • Assistive technology and specialist software and equipment used in supporting learners with SEND
  • Deaf/hard of hearing technologies and British Sign Language

Specialist Knowledge

Essential Criteria:

  • Specific knowledge and skills of working with learners with SEND/Learning Difficulties and/or Disabilities, EHCP, Specific Learning Difficulties (SpLD) which includes Dyslexia and Autism and SEND legislation
  • Strong knowledge of transition support e.g. from school to college
  • Strong knowledge of Education Health and Care Plans (EHCP)
  • Specific knowledge on High Needs funding
  • Ability to present and analyse information in a variety of forms
  • Ability to identity and use strategies to meet a range of student needs
  • In-depth understanding of the specific issues relating to the education of young people and adults who have learning difficulties/disabilities, sensory, physical or medical needs or other specific learning difficulties.
  • The ability to influence and improve aspirational learning support practice.
  • Ability to plan and produce quality learning resources to support learners to overcome barriers to learning.

Desirable Criteria:

  • Knowledge and experience of college business (e.g. structure, curriculum, support)
  • Knowledge of Exam Access Arrangements

Essential Criteria:

  • Advanced use Microsoft Office applications and willing to undertake training appropriate to role
  • Ability to use technology and applications to generate information and reports

  • Experience of EBS student records system, electronic ILPs, ProMonitor
